Colostomy Bag UK: The Complete Guide (Types, Changes & Living Well)

Colostomy Bag UK: The Complete Guide (Types, Changes & Living Well)

A colostomy bag — also called a stoma pouch — is a small, discreet pouch that collects waste after colostomy surgery. Whether you've just had surgery on the NHS or you're supporting a loved one, this guide covers everything you need to know: how a colostomy bag works, the different types available, how to change it, and how to live confidently with one.

What Is a Colostomy Bag?

A colostomy is a surgical procedure that diverts the colon through an opening in the abdomen called a stoma. The colostomy bag attaches to the stoma to collect faeces. Unlike the bowel, the stoma has no muscle control, so output happens involuntarily — the bag captures it hygienically and discreetly throughout the day.

Colostomies can be temporary (while the bowel heals) or permanent (when part of the bowel has been removed, for example due to bowel cancer, diverticular disease, or injury). Your NHS stoma nurse will advise which type you have and what to expect.

Types of Colostomy Bags

  • One-piece system: The pouch and skin barrier (flange) are combined in one unit. Simple and low-profile — popular for active lifestyles.
  • Two-piece system: The flange attaches to the skin and the pouch clips on separately. The pouch can be changed without removing the flange, reducing skin irritation.

Within each system, you can choose:

  • Closed pouches: Sealed at the bottom; disposed of when full. Best for formed stool output.
  • Drainable pouches: Open at the bottom with a clip or Velcro; emptied and reused. Better for softer or higher-volume output.
  • Transparent or opaque pouches: Transparent pouches let you monitor output; opaque pouches are more discreet under clothing.

Your NHS stoma nurse or Stoma Care Department will provide your initial supplies. On the NHS, stoma products are available via prescription.

How to Change a Colostomy Bag: Step by Step

  1. Gather your supplies: new pouch, barrier wipe or spray, stoma paste if needed, scissors, disposal bag, damp cloth or stoma wipes.
  2. Remove the old bag: Peel from the top down, pressing your skin gently inward to avoid pulling. Empty a drainable pouch first.
  3. Clean the stoma and skin: Wipe with warm water and a soft cloth. Pat dry thoroughly — moisture under the flange causes leaks.
  4. Measure your stoma: Stomas often shrink in the first 6–8 weeks. Use the measuring guide to cut or select the correct-sized hole (2–3mm larger than your stoma).
  5. Apply barrier protection: Use a barrier wipe or spray to protect peristomal skin. Apply stoma paste if you have irregular skin contours.
  6. Attach the new bag: Press firmly from the bottom up, smoothing out air bubbles. Hold for 30 seconds to activate the adhesive warmth.
  7. Secure and check: For two-piece systems, ensure the pouch has clicked securely into place.

Stay secure and confident. A well-fitted ostomy belt keeps your stoma pouch secure during exercise and everyday activity.

Managing Leaks

  • Wrong hole size: If the opening is too large, output sits against the skin and lifts the flange. Re-measure after weight changes.
  • Skin not dry enough: Even slight moisture prevents the adhesive from bonding properly. Pat thoroughly dry.
  • Irregular skin surface: Scars or skin folds create gaps. Try convex pouching systems or stoma paste to fill contours.
  • Overfull pouch: Empty closed pouches when one-third full to prevent the weight pulling the flange away.

Living Well with a Colostomy Bag

  • Sleep: Sleep in any comfortable position. Many people prefer closed pouches at night. Folding a small towel over the pouch adds reassurance.
  • Exercise: Physical activity is fully encouraged. An ostomy support belt keeps your pouch secure during sport and exercise.
  • Swimming: You can swim with a colostomy bag. Waterproof stoma pouch covers provide a secure seal and let you enjoy the pool or sea with confidence.
  • Intimacy: Stoma wraps and mini pouches are available specifically for intimate moments — discreet, comfortable, and designed to build confidence.
  • Travel: Always pack double the supplies you expect to need and carry some in your hand luggage. Under UK and EU law, airport security cannot require you to remove your appliance.

Diet and Your Colostomy Bag

Your diet affects the consistency and frequency of output. In the first few weeks, eat a low-fibre diet and introduce variety gradually. Foods that commonly cause excess wind include pulses, cabbage, and fizzy drinks. Foods that help thicken output include white rice, bananas, and white pasta. Aim to drink at least 2 litres of water daily to stay well hydrated.

Life Expectancy with a Colostomy Bag

Having a colostomy bag does not reduce life expectancy. Life expectancy depends on the underlying condition that required surgery, not the stoma itself. Many people live full, healthy lives for decades after colostomy surgery. The key is proper stoma care, regular check-ups with your NHS stoma team, and staying active.

When to Contact Your NHS Stoma Nurse

  • Your stoma changes colour (pale, very dark, or black)
  • Persistent skin redness, pain, or breakdown around your stoma
  • No output for more than 2–3 days
  • Prolapse (the stoma protrudes more than usual)
  • Persistent leaking despite correct fitting

Feel confident every day with your colostomy bag.
